CARRIE UNDERWOOD made the night -- and maybe life -- of a 12-year-old fan at a LOUISVILLE concert.

The young boy, CHASE, held a sign that read "Will you be my first kiss?" and was invited onstage for the big moment, which UNDERWOOD noted would no doubt be recorded and posted to YOUTUBE -- and it was.

"So how are we going to do this?" UNDERWOOD asked. But first warned the fan that her hockey-playing husband MIKE FISHER was in the house and "might be looking for ya later."

Not shy, CHASE answered "Lip to lip."

Following the lip lock, UNDERWOOD promised that that was "the only time that's gonna happen."

UNDERWOOD posted a thank-you on TWITTER after the show.
