You know, I never really thought of this, but someone thought to ask a TV station's "ask us" feature whether hospital workers are spreading germs when they wear their scrubs outside the hospital. And it turns out, no, probably not. If you're in surgery, yes, you'd be bringing bad stuff out with you, but people in surgery strip their scrubs off. If you're just working in the hospital, though, scrubs are like street clothes, and you're unlikely to be transmitting anything on them. But, still, ew.
