Thai Airways has reimbursed a passenger for charging him extra. What caused the charge? His name was too long for their website ticketing system, which meant his passport name and ticket name wouldn't match and he had to have an agent input it on the phone. This article also notes other reasons an airline could boot you from a flight, including B.O., refusing to wear shoes, being too fat for the seat (which is, unfortunately, not that hard to be these days, with seats getting smaller as people get bigger), and nut allergies.
